Job Summary Provides patients and family members with education and advice to cope with emotional or physical issues. Follows the Texas State Board of Social Worker Examiners scope of practice. Coordinates services like crisis intervention, bereavement, client advocacy, referrals, prevention, education, discharge planning, and psychosocial assessments. Partners in developing and implementing performance improvement initiatives. Essential Functions of the Role * Conducts and documents detailed psycho-social assessments/evaluations to interpret social, environmental, and medical needs of the patient/family. * Employs advanced practice methods in the development, implementation and evaluation of treatment plans. * Provides patient and family education and serves as a professional resource. * Provides or supervises care plans, including resources, referrals, crisis intervention, prevention, education, patient advocacy, bereavement, therapy, and counseling. * Partners with the healthcare team and involves the patient and/or family in the development and implementation of plans. * Serves as a liaison with community resources and staff to ensure smooth patient transition and placement within the assigned service line. * Partners with other healthcare team members to deploy the interdisciplinary care of patients and achievement of positive outcomes. * Participates in performance improvement initiatives to include data collection and review, development, and participation in continuing education programs. * Prepares and writes professional reports, letters, and case summaries. Key Success Factors * Knowledge of human behavior and performance includes differences in ability, personality, and interests. It also involves psychosocial methods and the assessment and treatment of behavioral and affective disorders. * Know the standards and methods to restore or enhance social, psychosocial, or bio-psychosocial functioning of people, couples, families, groups, organizations, and communities. * Listening and relational skills. * Verbal and written communication skills. * Skill in the use of personal computers and related software applications. * Advanced practice skills in the development, implementation and evaluation of treatment plans. * Ability to gather, record, and evaluate data. * Ability to help people in recognizing and solving problems. * Ability to handle grief. Belonging Statement We believe that all people should feel welcomed, valued and supported. *QUALIFICATIONS* * EDUCATION - Masters' * EXPERIENCE - Less than 1 Year of Experience * CERTIFICATION/LICENSE/REGISTRATION - Licensed Clinical Social Worker (LCSW), Licensed Masters Social Worker (LMSW), Licensed Master Social Work Advanced Practice (LMSW-AP): Licensed by the State of Texas as a Licensed Masters Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Work Advanced Practice (LMSW-AP).
